"Money Follows the Person" Looking For People in Shared Living Arrangements in the Aging Waiver

Hi All,

Through Money Follows the Person we are looking at ways to bring individuals from the State Hospital into the community. I am looking for information to help the Area Agency on Aging and the MH/MR agencies think outside the box.

The goal is for individuals being discharged to use Aging Waiver Services. Ideally, these individuals would go into shared living arrangements (because that has been their request). Does anyone have an example of a shared living arrangement in the Aging Waiver that they could tell me about? Specifically, we are looking at how the individuals could pool their resources to increase staff hours - thus having more staff present.

I have already outreached to one person and outreached to the nursing home transition team. I am also looking for some others. I know creative things have been done elsewhere, and I am trying to bring that creativity into this situation.
