Title V Needs Assessment Web Survey for Parents of Children with Special Health Care Needs
Title V Needs Assessment Web Survey for Parents of Children with Special Health Care Needs
REDA International, Inc. has developed a web-based survey that is directed toward parents of children with special health care needs. We would appreciate your help in getting the word out through your organizations to encourage parents to complete the survey. The link is provided below, along with some information that you can use to introduce the survey.
Pennsylvania's Bureau of Family Health is required to complete a statewide maternal and child health (MCH) needs and capacity assessment every five years. The purpose of the assessment is to gain a thorough understanding of the current health status of the MCH population groups (including children with special health care needs), their needs, and the Commonwealth's current ability to meet these needs. The Bureau has contracted with REDA International, Inc. to conduct the needs and capacity assessment that is due in 2010. They are collecting data through a variety of methods, including a web-based survey of parents of children with special health care needs. The survey is anonymous and will only take about 10-15 minutes to complete. They would like to hear from as many parents as possible from throughout Pennsylvania. To complete the survey, please go to http://www.redainternational.com/PATVHome.aspx and click on the box that says "TAKE SHCN SURVEY."
Thank you in advance for any help you can offer. Please let me know if you need additional information.
